Case Management: Our Philosophy

At Happiness at Home Health care, long term clients tell us apart from the relationships they develop with care givers, equally important is the relationship with our case managers. Clients say our case managers are friendly, polite, and respectful. Listening to the client’s needs and helping them with quality information and care supports regular care giving activities. Case managers have a keen sense of pairing the right care giver with a client.

If a client is returning from a hospital stay or a period of illness, a well informed case manager can expedite health and wellness, and eventually reduce the number of care giving hours a client requires (reducing care costs), helping the client to possibly regain their independence. Beyond day to day care insights, case managers have helped clients with:
